Current Phase of Vaccination Roll Out:

PHASE 1A ​&​ 65+ Population

Phase 1A include individuals who provide direct patient service (compensated and uncompensated) or engage in healthcare services that place them into contact with patients who are able to transmit SARS-CoV-2, and/or infectious material containing SARS-CoV-2 virus:

Hospital/Clinic healthcare workers
Long term care facility residents and workers
EMS
Physicians, nurses, and other medical workers not affiliated with a hospital system

65+ Population Eligible Monday January 25th:

Wisconsin Department of Health Services announced on 1/19 that adults over the age of 65 are eligible for the COVID-19 vaccine beginning January 25th. Sheboygan County health systems are preparing their plans to vaccinate the 65+ population and details on how to schedule appointments in Sheboygan County will be available soon.

**​WI DHS Announced Phase 1B populations with tentative start date of March 1st:

Eligible populations in Phase 1B will tentatively be eligible beginning March 1, though the start date is dependent on the vaccine supply from the federal government. Future eligible populations includes the following groups (in priority order):

Educators and child care
Individuals enrolled in Medicaid long-term care programs
Some public-facing essential workers like food supply, public transit, utility and communications infrastructure, and 911 operators
Non-frontline health care essential personnel
Congregate living facility staff and residents

Currently, the amount of vaccine allocated to Wisconsin does not meet the amount of vaccine doses requested by vaccine providers. The start date for vaccinations was determined given the current trend of Wisconsin’s weekly vaccine allocations. If the federal government increases our weekly vaccine allocation, vaccinations for these groups may begin before March 1. If Wisconsin receives less vaccine than expected in the coming weeks, this date could be pushed back.

Prevea/HSHS St. Nicholas Hospital:

Prevea Health’s Vaccination Clinic is currently located at HSHS St. Nicholas Hospital with plans to move to the University of Wisconsin-Green Bay Sheboygan Campus in early February.
Prevea is vaccinating those who meet the phase 1A and 1B criteria as determined by Wisconsin Department of Health Services.
Appointments should be made on MyPrevea.com; appointments are required and no walk-in appointments are availabl​e. You can also call 1-​833-344-4373 with vaccine related inquiries.
For those who do not currently qualify for the COVID-19 vaccine, Prevea encourages patients and members of the public to create a MyPrevea account at ​myprevea.com​ as this is where vaccine scheduling will be available.

Lakeshore Community Health Care:

LCHC is encouraging their patients and community members without a regular primary care provider (doctor) age 65 or older and those who may fall into an essential worker category to sign up to receive the COVID-19 vaccine at ​lakeshorechc.org/covid-19​.
Vaccines are currently limited. Registration will allow Lakeshore Community Health Care to plan for and distribute the vaccine as quickly as possible to those eligible, based on guidance from the Centers for Disease Control and Prevention (CDC) and Wisconsin’s phased distribution plan. The appointment takes about 30 minutes. No other issues will be addressed during the vaccine appointment. If you have other medical or dental concerns, please call 920-783-6633 to schedule a regular visit.
A team member from LCHC will reach out to you to schedule your vaccine appointment when you are eligible, based on Wisconsin’s phased distribution plan and vaccine availability. If you are over 65 or in phase 1A that could be within a few weeks. If you are not currently eligible, it could be months before you receive a call from LCHC. Thank you for your patience with this process.

Advocate Aurora Health:

Vaccine Clinic Location Change: ​Advocate Aurora Health (AAH) Vaccination Clinic is relocating from Aurora Sheboygan Memorial Medical Center to ​Aurora South Side Clinic located at 1813 Ashland Avenue, Sheboygan​ ​on Wednesday, January 27 with expanded hours and capacity.

Focused on Advocate Aurora Patients who are 65 and older:

Advocate Aurora Health (AAH) is scheduling vaccinations for AAH patients who are age 65 and older based on health risks.
We are contacting patients directly by email, text and phone when it’s their turn to schedule their vaccination, based on vaccine availability. There is no wait list at this time.
The goal is for everyone who wants a vaccine to be able to receive one in the coming months.
To prepare to receive a vaccination from Advocate Aurora Health, please download the LiveWell app on your mobile device from the Apple App Store or Google Play and create an account. Or, create a MyAdvocateAurora account at ​http://www.aah.org/myadvocateaurora​. Vaccine appointments will be scheduled through LiveWell and MyAdvocateAurora. If you do not have access to computer/email there will be a phone number to schedule your appointment.

Meijer Pharmacy:

Meijer has multiple ways that interested individuals can register with us for the COVID-19 vaccine:

Text ​COVID​ to the number ​75049​. They will receive a link to register.
Online at ​https://clinic.meijer.com/
Individuals without internet or smart phones can call their local Meijer

Pharmacy and speak with the pharmacy staff, who can assist with registration.

Sheboygan Internal Medicine Associates:

Sheboygan Internal Medicine has not received an allotment of vaccine from the state yet and are encouraging patients to utilize the other community options to receive the vaccine at this time.

Ascension Medical Group:

Ascension Medical Group Wisconsin (AMG-WI) has been reviewing medical records for all existing patients and identifying those who fit the above criteria. AMG-WI has been and will continue to proactively and personally contact eligible patients over the coming weeks by text, email and phone. ​Please do not call Ascension Wisconsin hospitals or clinics regarding vaccinations. AMG-WI will contact eligible patients.
Appointments for the vaccine are scheduled in advance. ​No walk-in vaccinations are available at this time​, but this may be adjusted in the future as additional vaccine doses become available.
Ascension Wisconsin is phasing the rollout to include additional Group 1B individuals based on CDC and DHS guidance and based on availability of COVID-19 vaccine. Ascension Wisconsin will continue to provide updates as additional vaccination opportunities become available. Thank you to the phase 1A providers who have reached out to Public Health. We are dedicated to reach all phase 1A providers in Sheboygan County. If your organization has not yet filled out ​this survey, please be sure to have one person from your organization complete the survey.
Public Health will match your organization with appointments as the vaccine becomes available, or match you with another vaccinator.

Action Steps for Individuals ​NOT​ in Phase 1A:

The timeline on when Phase 1B will be released is subject to change due to the amount of vaccine given to our community. The vaccine distribution process is complex and our allocations are subject to change. The anticipated numbers we will receive are determined by the federal government and our doses may change.
As the vaccine becomes more readily available the Division of Public Health will be providing information to the community through our ​Daily Update​ and will be keeping the Vaccine section of our website up to date.
Continue to follow the best practice recommendations of social distancing, staying home when you are sick, wearing a cloth face covering in public and handwashing.
If you have health conditions that put you at greater risk for COVID-19 complications, please contact your health care provider and they will work with you to get you vaccinated when the vaccine is available to you.
